Case Manager ALL LEVELS

Position Responsibilities:

Intake, assessment, service plan development/implementation, advocacy, and referral management and follow-up for people living with HIV and their family members. Coordinate and ensure access to services from multiple providers. Conduct home visits and escorts to appointments.

Qualifications:

Applicants must have a BA/BS, or HS diploma and 2 years’ experience. Bilingual Spanish/English a plus. Knowledge of/experience with, HIV/AIDS and addiction issues; family systems; advocacy with local, state and federal entitlement programs.

Fundraising Manager

Position Responsibilities:

Manage unrestricted fundraising efforts to reach budget goals, including prospect research, coordination of cultivation events, major donors solicitations, direct mail/internet campaign, and related strategies. Experience with Raiser’s Edge.

Qualifications:

Bachelor’s degree and minimum two years experience, or equivalent. Excellent written and verbal communication skills, computer literacy, ability to prioritize multiple responsibilities, meet deadlines, and work with a diverse range of people.

Program Manager, Client Services

Position Responsibilities:

Supervise COBRA case management teams, and ensure quality service provision, documentation and consistent staff performance on service targets. Perform QA activities and staff training.

Qualifications:

MSW required, plus minimum one year of supervisory and post-graduate experience working with individuals and families who have a history of substance abuse, mental illness, homelessness, and HIV/AIDS. Bilingual Spanish a plus.

Position Responsibilities:

Individual and group counseling on HIV Education and Treatment Adherence. Case conference with multiple providers. Conduct home visits and escorts to appointments.

Qualifications:

Bachelors Degree and minimum two years HIV experience, or equivalent. Knowledge of HIV treatment issues require. Bilingual a plus.
